If you are undergoing IVF treatment, you must provide the following official documents:
Personal Identification Documents
- Photocopy of both sides of your ID card or passport
Marital & Civil Status Documents
- Marriage Certificate (issued within the last 6 months)
- Family Status Certificate (issued within the last 6 months before egg retrieval/embryo transfer)
✅ If you are unmarried but in a partnership or single, you must provide:
- Notarial deed & Family Status Certificate (Please consult the clinic for details).
✅ If you have a civil partnership, you must provide:
- Registered Civil Partnership Certificate (issued within the last 6 months)
- Family Status Certificate (showing the civil partnership registration)
✅ If you were married outside Greece, you must provide:
- Officially translated Marriage Certificate (issued within the last 6 months).
✅ If you are a UK resident, you must:
- Obtain a certification from the British Embassy in Athens confirming that the UK does not issue a Family Certificate and Civil Status document.
Required Medical Tests (Valid within 3 months & signed by a doctor)
Both partners must be tested for the following:
- HIV I/II
- HBsAg (Australian Antigen)
- Anti-HCV (Hepatitis C antibodies)
- RPR (Syphilis Test)
For women over 35 years old, a mammogram or breast ultrasound is recommended.
Additional Tests (Upon Request)
These may be required based on medical history:
- TSH (Thyroid function test)
- Rubella (IgG & IgM) antibodies
- CMV (Cytomegalovirus IgG & IgM) antibodies
- Toxoplasmosis (IgG & IgM) antibodies
- Full vaginal culture test
- Hemoglobin Electrophoresis / Sickle Cell Test
- Complete Blood Count (CBC), Iron, Ferritin
- Blood Type & Rhesus Factor
- Semen Processing & Culture
- Cystic Fibrosis, Spinal Muscular Atrophy (SMA), 35delG mutation test
- Blood Karyotype test
