Natural Birth

Childbirth is the physiological process through which the fetus, placenta, and fetal membranes exit the mother's body via the birth canal. This occurs through rhythmic uterine contractions and cervical changes, including softening, thinning (effacement), and dilation.

Signs of Labor Onset

  • Reduced discomfort and easier breathing
  • Increased frequency of urination
  • Lower back pain
  • Stronger, irregular contractions (Braxton Hicks)
  • Increased vaginal discharge with blood-tinged mucus (bloody show)
  • Cervical ripening (partial effacement and possible dilation)
  • Possible rupture of membranes (water breaking)

Stages of Labor

1st Stage: From the Onset of Regular Contractions to Full Cervical Dilation (10 cm)
This stage consists of three phases:

  • Latent Phase: Mild contractions; the cervix dilates up to 3 cm.
  • Active Phase: Stronger, more frequent contractions; the cervix dilates from 3 cm to 7 cm.
  • Transition Phase: Most intense contractions; the cervix dilates from 7 cm to 10 cm.

The expectant mother’s behavior changes depending on the phase of labor.

  • Latent phase: Talkative and focused on herself and the baby.
  • Active phase: More serious and anxious, with pain evident on her face.
  • Transition phase: Cooperation becomes difficult due to intense pain.

2nd Stage: From Full Cervical Dilation (10 cm) to Birth of the Baby

  • Duration depends on previous deliveries:
    • First-time mothers: Can last up to 3 hours.
    • Mothers with previous births: Usually up to 2 hours.

3rd Stage: Placental Delivery (Afterbirth)

  • Maximum duration: 30 minutes
  • The mother can hold her newborn using the skin-to-skin method and initiate breastfeeding.
  • The umbilical cord is clamped after 1-5 minutes or when pulsation stops.

4th Stage: The First 1-2 Hours After Birth

  • The mother is monitored for postpartum bleeding, uterus height, blood pressure, temperature, and breathing.
  • The newborn is assessed and breastfeeding may be initiated if it has not yet occurred.

Pain in Natural Birth & Pain Management

Labor pain is subjective—each woman experiences and describes it differently in terms of intensity and sensation.

Pain Relief Methods

  1. Epidural Anesthesia:
    • The most effective and widely used method in private clinics unless medically contraindicated.
    • Requires informed consent after discussion with the anesthesiologist.
  1. Strong Analgesics:
    • Pethidine is sometimes used for pain relief.
  1. Non-Medical Pain Management Techniques:
    • Relaxation & Breathing Techniques (practiced in childbirth classes).
    • Movement & Position Changes (choosing positions that provide relief).
    • Massage: Gentle pressure on the lower back or legs, ideally by the partner or midwife.
    • Water Therapy: Warm showers or water birth to reduce pain perception.
    • TENS (Transcutaneous Electric Nerve Stimulation): A device with electrodes producing mild electrical impulses to manage pain, especially in early labor.
